The baby size is perfect for sleeping for a baby up to around 3 years old (so yes, it lasts a long time). After that, it can still be used for stroller rides, in the car, or on the sofa. (My three and a half year old son now uses the "medium" size in his bed for sleeping but still takes the baby size for outings)
